Why do so many institutions and systems function at suboptimal levels? According to this commentary, the ultimate culprits are aggregated interruptions and distractions, which add to an organization’s ‘marginal losses’. To minimize the corrosive effects of these losses, tailored mechanisms are necessary, as noted here.
